Northampton County Says Gracedale Is Not Closing After Provisional III License Change

Northampton County, Pennsylvania | November 7, 2026. County Executive Tara Zrinski says Gracedale Nursing Home is not closing following a change in the state license status of the county-operated facility. The statement follows September inspections and a new state licensing determination.

What changed

According to the county’s November 6 announcement, the Pennsylvania Department of Health rescinded Gracedale’s Provisional II license and issued a new six-month Provisional III license. The county says this restarts a six-month period for addressing deficiencies. Its announcement acknowledges additional violations despite earlier improvement efforts.

What the county says it will do

The county must submit an acceptable Plan of Correction and demonstrate that corrective steps are working. It also plans to bring in an outside consultant to examine facility policies, procedures and resident care plans. These are announced next steps, not proof that the deficiencies have been resolved.

What residents and families should know

Gracedale serves residents from across Northampton County, including the northern part of the county around Walnutport. Families with questions about care or a particular resident should contact the facility directly. Official county and state notices should be used to follow licensing changes; the term Provisional III does not itself mean closure has been ordered.
